Once you have completed this form you will be sent an email with a link to your returns label. Please print out this label and attach it to your returns parcel as it displays your unique reference number.
We have requested photographic evidence to show the product has been decommissioned (we sent you instructions with our original email to you). Should you need to see those instructions again, please download them now….

Once either decommisioning evidence (photo) or product is received, we will either automatically arrange an FOC replacement shipment and will send details of shipment date, or contact you to discuss further details. Please allow us 48 hours to process your return and make replacement shipment arrangements.
